In medicine, sodium thiosulfate is a key antidote for cyanide poisoning, where it acts as a sulfur donor to convert toxic cyanide into the less harmful thiocyanate, which is then excreted in the urine. It is typically administered intravenously after sodium nitrite and is recommended for severe cases. It is also used to reduce the risk of ototoxicity and hearing loss in children and adolescents receiving cisplatin chemotherapy; the U.S. FDA approved it under the brand name Pedmark in September 2022 for this purpose, and it is similarly approved in the European Union as Pedmarqsi. Additionally, it is used topically to treat fungal skin infections such as pityriasis versicolor and ringworm. There is also emerging evidence supporting its use in treating calciphylaxis, a rare and serious condition involving vascular calcification in patients with kidney failure, where it may help dissolve calcium deposits.

In industrial and environmental contexts, sodium thiosulfate is a powerful dechlorinating agent, used to neutralize chlorine and chloramine in water treatment, swimming pools, aquariums, and wastewater systems. It is also used in the photographic industry as a "fixer" to remove unexposed silver halides from film and prints, stabilizing the image. In mining, it serves as an alternative lixiviant to cyanide for extracting gold from refractory ores, forming a soluble gold-thiosulfate complex that can be recovered. It is also employed in the textile and paper industries to remove excess chlorine and in leather tanning. Furthermore, it functions as a reducing agent in chemical synthesis and as a titrant in iodometric titrations for analytical chemistry.

Sodium thiosulfate is also used as a food preservative, typically added to table salt and alcoholic beverages at low concentrations. It has been explored for potential neuroprotective and anti-inflammatory effects in conditions like Alzheimer's and Parkinson's disease, although these applications are still under investigation.
